- Pandemic Influenza Planning Grants Information - County and Municipal COOP Template - County and Municipal Fatality Management Plan Template

Most of you attended the Pandemic Influenza Grants Workshop held June 12, 2008, in Clanton, Alabama. Appreciation is expressed to all who attended and for the enthusiasm and willingness to participate exhibited by all. As promised, included herewith are the two templates and other information necessary for you to receive the grants. Please review the following and follow any instructions provided.

1.

COOP/Op Plan Template The enclosed template must be completed in its entirety with the exception of the few provisions which are indicated as optional or not necessary. Once completed the template must be returned to the address indicated below.

2.

Fatality Management Plan Template The enclosed template must be completed in its entirety. (There are no exceptions). Once completed the template must be returned to the address indicated below.

3.

Documentation Form The enclosed documentation form must be completed and returned with the two completed templates. Please notice that the form must be signed by both the local EMA Director (or other responsible person) and the County Commission Chairperson.

Grant Award Requirements Requirements for receiving the grants are as follows: (a)

Completed templates and documentation forms must be returned no later than November 1, 2008.

(b)

Templates must be approved by the appropriate state-level representatives

of ADPH.

5.

Permissible Uses of Grant Proceeds Grant proceeds may be used for reasonable program purposes including personnel, travel, supplies and services, such as contractual. Grant proceeds may not be used for construction or major renovations, fund raising activities or political education and/or reimbursement of pre-award costs.

6.

Grant Awards Grant awards will be made as soon as possible after templates have been reviewed and approved. It is anticipated that grant checks will be distributed within 15 days after receipt of completed templates. All grant checks will be made payable to the county commission.
