Patient Information Insurance information: We accept most insurance. Please contact the office directly if you have questions regarding your specific insurance plan. Payment information: For patient convenience we accept cash, checks, and credit cards (MasterCard, Visa, and Discover) for co-pays, deductibles, and insurance co-insurance at time of service. Appointment Information: Please note that we do not see patients on Friday afternoons. Patients are only seen by appointment. Some same day appointments may be available for sick visits. Current patients may request appointments through the online Patient Portal and we will call you to confirm your appointment. Please bring your insurance card and driver’s license to each appointment. We value each patient’s time and strive to see each patient at the time of his or her scheduled appointment. Appointment slots are valuable and we ask patients to give us a minimum of 24-hours advance notice for cancellations. Please note that patients are not permitted more than three no show appointments.
