Hepatitis B Shots ( Series of 3)

Please be advised the attached lists of employees have signed up to have the Hepatitis B shots. The cost of the 3 series shots $120.00 for each employee. There are approximately 75 interested employees who have signed up. The cost will be around $9,000.00.

This Department is requesting funding from the Board of County Commissioners. These funds were not allocated in this years' budget. These shots were recommended during the last two Safety Committee meetngs by all in attendance. The shots will be given by the Columbia County Health Department.

Good news! The Lake City Mall approved our request to have the Columbia Community Traffic Safety Day behind the Mall on Saturday, March 24 from 10 a.m. to 2 p.m. We can use the paved parking lot and the grassy area next to it. The Mall manager said this is the same day that the Easter Bunny arrives and it will bring in even more traffic to the Mall that day. She will also advertise our event on their marquee that faces U.S. 90. Since we are holding it on Mall property, there is a hold harmless agreement that each agency must sign in the event something happens at our event. The agreement is attached if you can start the process to get it signed by your agency.

Also, the Mall will make a poster for us advertising the event and if we send them a logo, she can add it to the poster. Sounds like a win-win partnership with the Mall and looking forward to bringing this public service event to Lake City on March 24. Please let me know if you have any questions.

I am requesting a variation of policy to exclude recruitment for the four (4) positions on the urbanized mowing crew. This crew has been on board as temporary labor through Wal-Staff since October and has performed exceedingly well. I do not believe it would be in the County's best interest to spend additional funds for recruitment for these positions when we have folLr (4) very good candidates available currently. I would ask that all other requirements be met, ie. license check, background check, drug testing, agility testing, etc.

The four positions would be classified as: (1) crew leader and (3) maintenance technicians with the job duties the same as those in Landscape and Parks. If you need any additional information, please contact myself or Connie.

FIRE CHIEF

MAJOR FUNCTION: Under administrative direction, plans, directs, and supervises the activities of the Columbia County Fire Department in the protection of life and property. This position has direct supervision over all department personnel and reports directly to the County Manager. Work is reviewed through conferences, reports and observation of departmental operations. Successful performance helps ensure the protection of community life and property and affects local insurance rates.

ESSENTIAL FUNCTIONS: Plans, directs, and coordinates all department operations, manages the provision of fire prevention and suppression services to the County. Responds to severe fire calls, and assumes command of extreme emergency operations. Is on call at all times. Develops the annual departmental budget, monitors expenditures under the current budget, approves equipment requisitions, and oversees an inventory system for equipment and supplies. Prepares a five-year operations plan for the department. Submits or presents reports to the County Commissioners and the County Manager. Ensures that all departmental equipment is in proper working order. Responsible for selection; training, evaluation, promotion, and discipline of departmental personnel. Commands and coordinates emergency incident activities to include multi agency operations. Attends training serriinars, workshops, conferences and conventions to remain abreast of current trends in departmental and operational management. Performs fire safety and public safety education and acts as an instructor for in service fire department training. Participates in Fire Department related community activities as assigned. NON-ESSENTIAL FUNCTIONS: Performs other related duties as assigned. WORK ENVIRONMENT: The work is typically performed in an office, except while at scene. The employee is exposed to noise, dust, and dirt, machinery with moving parts, irritating chemicals, extreme temperatures, hazardous situations, and occasional inclement weather. The work requires the use of protective clothing and devices.

KNOWLEDGE SKILLS AND ABILITIES: Knowledge of fire-fighting standards, practices, strategies, tactics, and equipment. Knowledge of and/or willingness to learn the geography, streets, buildings, and fire hydrant locations of the County. Knowledge of first aid principles and applications. Knowledge of budgeting practices. Knowledge of hazardous materials. Knowledge of National Incident Management Systems. Skill in management and supervision under both routine and emergency situations. Skill in decision making and problem solving.

Skill in oral and written communication and interpersonal relations. Skill in the operation and use of fire equipment and apparatus. Skill in developing and implementing long-term plans. Knowledge of professional guidelines including county and state fire codes, National Fire Protection Association guidelines, local ordinances, and departmental rules and regulations. These guidelines require judgment, selection, and interpretation in application. The employee develops new guidelines as needed. Ability to maintain professional relationships with elected and other local government officials, other department heads, representatives of emergency assistance agencies, employees, and the general public.

TRAINING AND EXPERIENCE: Must obtain the required minimum documented training as set forth by Florida Law. Must obtain the required training as set forth by the Columbia County Fire Department. Must obtain and maintain a Florida State Firefighter Certification within first year. Bachelor's degree in a related occupational field and/or ten (10) years of progressively more responsible authority in the fire service with at least two (2) years service as battalion chief or accistant fire ch leí. Ability to maintain or lower the ISO class rate held. Must understand the ISO process and its im pact.

ESSENTIAL PHYSICAL ABILITIES! DEMANDS: The work is performed with the employee sifting, standing, walking, bending, crouching or stooping. The employee must be able to lift heavy objects (at least 75 pounds), climb ladders, use tools or equipment requiring a high degree of dexterity, and be able to distinguish between shades of color. Importance: High Good afternoon! The North Florida Economic Development Partnership (NFEDP) is coordinating the execution of a Second Amendment to the Second Memorandum of Agreement (MOA) relating to the Columbia County and Suwannee County Catalyst Sites which was originally executed by the 14 counties Boards of County Commissioners in July 2008. When the 2nd MOA was prepared, a Scribner's error occurred regarding the Suwannee County parcel identification numbers which has now been corrected and incorporated in this MOA. A second unforeseen issue has arisen which is also addressed in the 2nd Amendment to the 2nd MOA. In order for the Columbia County and Suwannee County Catalyst Sites to compete against neighboring, highly competitive states, Columbia and Suwannee Counties need to be able to offer ad valorem tax abatement as part of their economic development toolbox incentives to recruit prospects to locate at the Catalyst Sites. Columbia and Suwannee County have the authority by referendum to abate those taxes and each county is currently working with prospective companies which are also considering other states in the Southeast US. Without the abatement, the companies will not locate in Florida. After much consideration, the counties have concluded that the most important goal is to create jobs for the NFEDP region and the small amount of tax revenue distribution which was outlined in the 2n MOA is insignificant compared to potential job creation currently being considered at the two Catalyst Sites. Therefore, this issue is also included in the 2 Amendment to the 2nd MOA. This issue was addressed at the July 2011 NFEDP Board meeting, a motion was made and passed unanimously by the full board. However, to make sure all BOCCs are aware of the issue, it ¡s addressed in the 2' Amendment to the 2nd MOA.

Once the MOA is executed and returned, the Department of Economic Opportunity (DEO) Director will execute and return it to the Institute of Government at FSU. One copy will be emailed to you for your files and one will remain on file at DEO.

January 19, 2012 The Board of County Commissioners met in a regularly scheduled meeting at the School Board Administration Office. The meeting opened at 7:00 p.m. with prayer. The Pledge of Allegiance to the Flag of the United States of America followed. Commissioners in Attendance:

Chairwoman Scarlet Frisina, Stephen Bailey, Russell "Rusty" DePratter, Ronald Williams, and Jody DuPree.

Others in Attendance:

County Manager Dale Williams, Senior Staff Assistant David Kraus, Asst. County Manager Lisa Roberts, and Deputy Clerk Sandy Markham.

Additions to the Agenda: Happy House's request for dirt donation

Deletions from the Agenda Happy House's (representing multi agencies) request for temporary transportation subsidy funding Discussion to purchase forty-eight acres adjacent to Target Ring Power Contracts MOTION by Commissioner Williams to approve the amended agenda. Second by Commissioner Bailey. The motion carried unanimously.

Public Comment No requests.

Bea Coker's Florida Crown Issues Attorney Feagle stated that it is his understanding the issues that Ms. Coker will address with the Board are currently in an administrative quasi-judicial process. Therefore, he recommended the Board allow her to address her issues, but asked that the Board refrain from commenting. Attorney Feagle also advised Ms. Coker that the meeting was being recorded and may be obtained and used in any future hearings. Ms. Coker discussed her complaints alleging Florida Crown has violated her civil rights and has discriminated against her. She asked the Board to not allow citizens to be mistreated. Miscellaneous documentation is attached to original minutes.

Consent Agenda Additional clarification was offered on item #11 by County Manager Williams.

MOTION: By Commissioner Williams to approve. Second by Commissioner DePratter. The motion carried unanimously.

Nonexciusive Solid Waste Franchise Application and Agreement Staff recommended the following:

No limit on the number of non exclusive franchises issued Do charge an application fee Do not charge franchise fees

MOTION: By Commissioner William to approve staff recommendation. Second by Commissioner Bailey. The motion carried unanimously. Application Changes Recommended by Staff

Delete from Section II, Paragraph C. "Verfìcation ofsource of load and weight ticket shall be required by the County on a quarterly basis." Delete all points from Section II, Paragraph D, and have only the following wording: "The County shall require Franchisee to submit additional information as the county may reasonably require to verfj' applicant is adequately prepared tofu/fill the Franchise Agreement." Section III, Paragraph I, sentence should read: "The franchise shall be for a one year period and is renewable for successive years thereafter on the anniversary date of the originalfranchise." Inclusions to the Agreement as Directed by Commissioners:

The verbiage "adequately prepared" is too subjective. The word "adequately" is to be clearly defined and the expected "standards" will be spelled out in the agreement. The County shall be named as an insured party in the Franchisee's policy and will be notified if insured's policy is about to lapse.

MOTION: By Commissioner DuPree to approve staff recommendations for Section two subject to changes discussed by Commission. Second by Commissioner Bailey. The motion carried unanimously.

Application Fee The initial term of application will have a fee of $500. Annual renewal fees for successive years shall be $50 per year.

MOTION: By Commissioner DuPree that the franchise be for a one year period and renewed yearly thereafter as desired. The origination fee shall be $500 and the renewal shall be $50 per year thereafter. Second by Commissioner Williams. The motion carried unanimously.

Section II, Paragraph C. Staff asked if it is the intent of the Board that nonexclusive franchise holders are to carry the materials collected within this county to Columbia County's landfill or to another place designated by the Board. MOTION: By Commissioner Williams to require "all" waste of all types and classifications that are generated in this county be disposed of in Columbia County only. Second by Commission DuPree. Commissioner DePratter is of the opinion that if lower prices for disposing can be found outside of the county, it is a savings to the taxpayer. He said that it appears to him that the burden is being placed on the businesses that are here to make money on the citizens. The motion carried 4-1 with Commissioner DePratter voting in opposition. Attorney Feagle noted the contract will need to reflect and "ending" date. MOTION by Commissioner Williams to authorize Attorney Feagle to add language so that the Board is not required to renew a contract even if conditions are met. Second by Commissioner DuPree. The motion carried unanimously. Attorney Feagle offered a letter into the record that he wrote to the Board of County Commissioners on January 19, 2012. The purpose of the letter was to supplement and clarify his letter to them dated January 11, 2012. The letter addressed the county's position regarding solid waste collection. As well it discussed Waste Pro's requesting a franchise, and the efforts underway to amicably resolve outstanding issues between Waste Pro and Columbia County.

Happy House Donation MOTION: By Commissioner DuPree to approve Happy House's request for a donation consisting of two loads of dirt [Value $224] to be used on the playground. Second by Commissioner DePratter. The motion carried unanimously.

Lifestyle Enrichment Center Commissioner Williams would like for the Board to look into the possibility of helping this facility open 5 days instead of four to benefit the seniors. This request will also include the Fort White Seniors and their facility.

Water Bills Commissioner DuPree reported that Senator Dean's water bill has passed both committees, and Representative Porter's bill has passed one committee and should be before the second committee next week. It appears both bills are floor bound.
