Key Terms •

Co-insurance – the percentage of Eligible Expenses in addition to the Premium, which Covered Persons are required to pay for certain Health Services provided under the Policy. Covered Persons are responsible for the payment of any Co-insurance directly to the Provider of the Health Service at the time of service or when billed by the Provider.

Deductible - a fixed monetary amount paid by the insured member to the Network Provider for any defined benefit at the time of the service or when billed by the provider, as mentioned in the Schedule of Benefits.

•

Network Provider: a provider of Health Services in an agreement with Daman - hospitals, single and multi-specialty clinics, pharmacies, diagnostic centres, etc.

How To Obtain Your Medical Benefits How to use your Daman Card •

Present your Daman health insurance card at the Network Provider’s reception.

•

Comply with the Network Provider’s administrative procedures.

•

As a Daman Card holder, there is no need to pay for covered services attained from the healthcare provider within Daman’s network, except for those offered on reimbursement basis in case of emergency (deductible would be applicable as mentioned on the card).The Network Provider should inform you in advance in case there are deductibles or if any of the prescribed procedures or medical treatment is not covered by your health insurance plan.

•

You will be asked to settle co-payments or deductibles (if mentioned on your card) either before or after the treatment.

•

The Network Provider will provide you with all the necessary information, if needed.

Pre-Authorization A pre-authorization is a prior approval or official permission, given by Daman to the Network Provider, before certain procedures. This approval is required to support the Provider’s choice of medical procedures and avoid any unnecessary procedures, hospital stays, or surgeries.

Your Network Provider will inform you if a requested or prescribed procedure requires preauthorization. To facilitate and speed the pre-authorization process, always verify with the Network Provider whether the planned services are covered under your Daman health insurance plan. The Network Provider, along with Daman, will carry out the pre-authorization process. Once the authorization is granted, the Network Provider will notify you. In case the authorization is not granted, then you can always request for an explanation. There is always a chance that some services might not be covered, even if they have already been performed or prescribed (unless they were authorized prior to being performed).

When is Pre-Authorization Required? Pre-Authorization is required for all inpatient and day treatment services. For outpatient services, pre-authorization is required in case of the following services: •

Physiotherapy

•

MRI, CT Scans and Endoscopies

•

Prescription of long-term medication above 60 days for Basic Plus Plan and prescription of long term medication above 30 days for UAE Optima and UAE Limited Plans

•

Dental Treatments (if assured in the Schedule of Benefits)

Emergency cases that are followed by a hospital admission do not require pre-authorization from Daman but have to be notified by the Network Provider to Daman within 24 hours.

Reimbursement In case of treatments that have been availed outside Daman’s Network, you will have to pay for the services provided. However, if such services are covered in your plan, you can apply for reimbursement.

All reimbursement claims, for Health Services availed in the UAE, should be reported to Daman within 120 days from the date of occurrence, and should be submitted with the following required documents: •

Original itemized invoices with dates

•

Medical Report/Discharge Summary signed and stamped by the treating doctor (for inpatient services only)

•

Original drug prescription with a detailed pharmacy bill (itemized)

•

Diagnostic (lab/radiology) investigation reports and invoices (if any), report of the results only if single investigation cost is more than AED 1,000

Reimbursement claims submission can be applied online through Daman’s web (www.damanhealth.ae) or through Daman’s interfaces, either in branches or service points.

Reimbursement forms are available on Daman’s website. Reimbursement of appropriately documented claims (as per the list of documents above) will be settled within 15 working days from the date of receipt of the claim, and a cheque will be issued in the policy holder’s or principal’s name along with a detailed settlement report.
