1.
Introduction
1.1. This document specifies basic qualification requirements that the candidates shall meet to become qualified suppliers for SE, a.s. for contracts in the field of “TRANSFORMERS – maintenance, repair”. The qualification (inclusion into the qualification system) will be successful in case that the candidate fulfilled all requirements defined in the General qualification terms (GQT), as well as requirements defined in this documentation (Documentation). GQT form a part of the Documentation in a form of separate document. Amendment referred to in the source documents shall take precedence over that specified in the GCQS.
1.2. The qualification system is announced for maintenance, revisions and reconstruction of oil and dry transformers. Transformers are installed in technological systems in the power plants buildings. It serves for the outlet of power from alternators as well as ensuring own consumption and distribution of electricity for own consumption. The qualification system is kept for categories: 1. Nuclear power plants - including controlled area - works in the environment with ionising radiation Slovenské elektrárne, a.s., Bohunice Nuclear Power Plant, 919 31 Jaslovské Bohunice (EBO). Slovenské elektrárne, a.s., Mochovce Nuclear Power Plant, 935 39 Mochovce (EMO).
2. Thermal power plants and hydro power plants Slovenské elektrárne, a.s. hydro power plants Trenčín, Soblahovská 2, 911 69 Trenčín (VET). Slovenské elektrárne a.s., Nováky power plant, 972 43 Zemianske Kostoľany (ENO). Slovenské elektrárne, a.s., Vojany Power Plant, 076 73 Vojany (EVO).
1.3. The following subcategories are held for the qualification system categories: - dry transformers : 6/0,4 kV ( outputs from 250 kW to 1600 kw ) - oil transformers (e.g. 250MWA – 400/15 kV, 64 MVA – 110 / 6,3 kV, 32 MVA – 15 / 6,3 kV, 125MVA - 400/13,8 kV, 242/13,8 kV, 110/13,8 kV, 25MVA -110/6,3kV, 16MVA13,8/6,3kV, 250kVA-22/0,4kV, 40MVA – 110/22 kV; 10MVA-22/6,3kV, ) ... 1.4.
The candidate will be qualified in the respective category depending on the subject-matter of the contract,
evaluating submitted quality certificate,
evaluating submitted references.

2. Description of the Subject from the Material Group
2.1. The subject of the material group for which the candidates are qualified is the maintenance, revisions, reconstruction of dry and oil transformers. 2.1.1
Maintenance, revisions and reconstruction of dry transformers:

inspection, repairs and new insulations of transformers winding,
-
inspection, repairs and new insulations of transformer bank of sheets,
-
measurement of electric quantities at transformers
-
transformers reconstruction
2.1.2
Maintenance, revisions and reconstruction of oil transformers:
-
inspection of winding and current connections,
-
inspection and new insulations of transformer bank of sheets,
-
revision of winding branch switches,
-
inspection and repairs of the tightness of connections at transformers pressure vessel,
-
inspection of revisions of transformer bushings and current and voltage metering transformers, neutral point
-
revisions and repairs of accessories - pressurizer, Bucholtz relay, cooling including pumps and ventilators, control cabinets and automatics, encapsulated outlets,

electric measurement at transformers,
-
treatment and purification of transformer oils including chromatographic oil analyses..

2.2. The contractor shall perform works as follows: independently, covering all areas with respect to own procedures or technological procedures recommended by a producer, and presented design documentation or SE’s technical documentation. The contractor shall perform activities in their own premises with equipment for the execution of revisions and repairs of big oil transformers, transformer oil handling and purification. The Contractor shall have available measuring apparatus for high voltage measurements. 2.3. If the contract is delivered in the NPP controlled area, conclusion of the contract with the winner is subject to the submission of following certificates for all qualified employees: 2.3.1.
Certificate of health, mental competence and impeccability in accordance with Act No. 541/2004 Coll. on Peaceful Use of Nuclear Energy (Atomic Act) § 26 Sect. 5,
2.3.2. Document of impeccability – a valid abstract from the criminal record no older than three months, 2.3.3.
Document proving health capability – medical certificate issued by the medical doctor authorised to issue such certificates,

Document proving mental capability – certificate about successfully passed psychological tests issued by a psychological centre authorised to issue such certificates,
2.3.5.
Certificate of competence in accordance with Act No. 355/2007 Coll. as amended on the Protection, Support and Development of Public Health pursuant to §15 Sect.1 Letter c) d) e) in compliance with §45 of the respective act, required for the 1st category NPPprimary circuit. In case persons that will ensure the qualification system subject have valid access to SE, a.s. with regard to which they provided proves of necessary trainings for works in NPP issued by VÚJE, criminal integrity, health and mental capability, it is not necessary to submit these documents repeatedly.
3. Qualification Requirements 3.1. Basic requirements the candidate is obliged meet in order to fulfil the qualification terms and conditions are divided as follows:
personal status conditions; economic and financial standing conditions; Requirements on technical and professional competence, and quality assurance.
Personal status conditions, economic and financial standing conditions, as well as the way of submitting and proving of qualification requirements by the candidates for this qualification system is defined in the GQT.

3.2. 3.2.1.
Requirements on technical and professional competence, and quality assurance. The candidate fulfils technical and professional pre-conditions, quality assurance for performing contract if: -
they are experienced in the delivery of similar subjects it disposes of a qualified personnel, technical equipment and organisational structure for the delivery of similar subject-matters; it has a documented and functional quality management system for the given subject pursuant to ISO 9001. it has an occupational health and safety (OH&S) system documented and functional according to standards OHSAS 18001:2007; it has an environmental management system documented and functional according to ISO 14001 or EMAS.

The candidate shall prove its technical and professional capability by submitting the following documents in the system myHome, depending on qualification in various qualification categories and subcategories pursuant to Clause 1.2. a. References, at least one, of the same or similar character, performed during last 5 years, in each category and subcategory for which the candidate is qualified, within the subject of the material group. References from the category 1 will be accepted as references fulfilling the requirements from the category 2. . The candidate shall submit information on performed contracts (references) on a form, template of which will be available after registration to the relevant qualified material group in myHome. Execution of each contract shall be confirmed by the customer or document certifying the subject of the contract delivery (contract, takeover protocol). SE is entitled to verify the facts provided in the List of Contracts (references) at the candidate’s place. b. Contractor’s certificate to perform activities pursuant to Act No. 124/2006 Coll. according to § 15 (1) for VHV equipment and Decree No. 508/2009 Coll. § 19 for all categories and subcategories. c.
A quality certificate related to the corresponding subject of qualification issued by an independent institution according to requirements of EN ISO 9001 or other document (e.g. Quality Manual) proving that the candidate has a qualified management system implemented according to EN ISO 9001 requirements.
d. An environmental management system certificate issued by an independent authority according to EN ISO 14001 requirements, or other document (e.g. by a registration in EMAS) proving that the contractor has an environmental management for the subcategory oil transformers for the both categories. e. OH&S management system certificate according to OHSAS 18001 standards. When candidate doesn´t have certified OH&S management, he has to fill in the questionnaire „HSE checklist“ which is stated in the attachment of this documentation, by which SE, a.s. consider OH&S management functionality. Completed questionnaire together with the documents and information required in the questionnaire the candidate submits in myHome on the form „Documentation“ in the second part of the qualification questionnaire. Candidate has to achieve 14 points from the total 28 points by the HSE checklist evaluation for qualification. f.
A statement on audit performance or acceptance of external audit by SE, a.s. The candidate qualified for the 1st category and both subcategories will submit a solemn statement about the audit execution giving its number, date and duration. If the candidate was not audited or the audit certificate validity expired, the candidate will submit a solemn statement that the candidate and its contractor agree with its execution.

The audit purpose is to verify information obtained about the candidate, and to check a quality management system’s functionality by qualified SE personnel. The contractor, the quality management system of which was audited by SE, will be included in a List of Audited SE Contractors at a respective level during the audit validity term. g. A list of machinery and hardware, for all categories and subcategory oil transformers, the candidate has at disposal for execution of the contract subject. The list must contain the equipment name, its description and relation to the candidate (ownership, operative leasing, and rental). The contractor shall list all production workshop, which are at their disposal and which are equipped with suitable facilities so that they are able to implement production referred to in the subject of qualification. The Contractor shall have available premises for the execution of revisions and repairs of big oil transformers, transformer oil handling and purification system, measuring apparatus for high voltage measurements. SE, a. s. shall be entitled to verify the facts stated in the list by audit at the contractor or owner or tenant. The Contractor shall state in the list that in case of auditing, they agree with it and ensure the audit performance at the owner or tenant if they do not own the production workshops.
