Inpharma 1231 - 1 Apr 2000 DOTS strategy effective in preventing TB resistance The WHOâs recommended tuberculosis (TB) control strategy, Directly Observed Treatment, Short-course (DOTS), has prevented the increase of drug resistance in countries using the strategy, according to a report from the Global Project on Anti-Tuberculosis Drug Resistance Surveillance entitled Anti-Tuberculosis Drug Resistance in the World.* The DOTS strategy is aimed at ensuring the correct provision of medicines by health services and ensuring that patients complete the treatment course without interruption. The report reveals that not only have countries using DOTS prevented the increase of drug resistance, but in Cuba and Nepal, levels of drug resistance have started to decline. Regions in China where DOTS is used have drug resistance levels 3 times lower than those in regions where DOTS is not used. DOTS is the first of the WHOâs strategies to stop the spread of drug resistance. This strategy is designed to correct careless treatment practices and thus prevent resistance from developing; the WHO is developing its second strategy, known as DOTS-Plus, to deal with problems resulting from careless practices in previous TB treatment programmes. The third strategy is to encourage development of new antitubercular drugs and vaccines. * The report was prepared by the the Global Project on AntiTuberculosis Drug Resistance Surveillance, which is headed by the WHO and the International Union Against TB & Lung Disease.
