INTENSIVE PROBATION ORDER Conditions of Probation pursuant to Illinois Compiled Statutes Chapter 730, Section 5/5-6-3 On This Day, defendant herein, having been convicted of the offence of _________________________________ _______________________________________________, and the Court being duly advised in the premises: It Is Hereby Ordered that the defendant be sentenced to a term of probation for ____ months, and that as a condition of said probation, the defendant: 1. Shall not violate any criminal statute of any jurisdiction; 2. Shall report to and appear before the Will County Adult Probation Officer this date and at such other dates and times as directed by the Officer; 3. Shall refrain from possessing a firearm or other dangerous weapons; 4. Shall not leave the State without the consent of the Court, or in circumstances in which the reason for the absence is of such Emergency nature that prior consent by the Court is not possible, with the prior notification and approval of the Probation Officer; 5. Shall permit the Probation Officer to visit at the defendant’s home or elsewhere to the extent necessary to discharge the Probation Officer’s duties; 6. Shall notify the Probation Department and the Clerk of the circuit Court of any change of address within seventy-two (72) hours; 7. Shall in addition to the above conditions, serve the initial period of probation under the supervision of the Intensive Probation Supervision Unit, and abide by all the rules, regulations, and policies of that program, with the specific rules and regulations of Intensive Probation being that the defendant shall; a. Perform and complete 130 hours of Public Service Work, and if unemployed and/or not attending school, 20 hours of Public Service work per week, even if said 130 hours have been completed. Transportation shall not be provided; b. Remain at one place of residence and not move from that residence without permission of a Probation Officer; d. Submit to random urinalysis testing at the request of a Probation Officer and pay the cost of said testing; e. Submit to a search of person, residence, papers, automobile or effects at any time as directed to do so by a Probation Officer; f. Attend and participate in counseling, treatment, or educational programs as directed by a Probation Officer, and abide by all rules, regulations and directions of said programs: g. Comply with all curfew conditions established in writing by a Probation Officer requiring the defendant to be and to remain at certain places during certain times; h. Not be present at any location which has been prohibited in writing by a Probation Officer; i. Cooperate with and maintain truthful, accurate and appropriate communications with Intensive Probation Officers and promptly, fully and completely obey all orders given by any Probation Officer; j. Make regular payments toward costs, as suggested in writing by the Probation Officer; 8. Shall obtain a drug & alcohol evaluation and follow the recommendations made therein. 9. Shall be personally present before this Court for status hearing on ____________________, and future status dates set therein. 10.
