Zusammenfassung. Die Bedeutung der laparoskopischen Chirurgie zur operativen Behandlung benigner gastrointestinaler Erkrankungen nimmt immer mehr zu, wohingegen diese operative Vorgehensweise bei malignen Erkrankungen weiterhin sehr umstritten bleibt. Obwohl die Lokalisation von Tumormetastasen im Bereich von Wundrändern an sich nichts Neues darstellt, so ist doch das mögliche Auftreten von Metastasen im Bereich der Trokareinstichstelle der Haupteinwand gegen das laparoskopische Operieren von Malignomen. In diesem Zusammenhang ist es notwendig, Indikationen und Kontraindikationen sowie das standardisierte laparoskopische Procedere genau zu definieren. Alleinige laparoskopische Techniken oder kombiniert laparoskopisch/offene Verfahren bieten jeweils Vor- und Nachteile, die der Chirurg in seine therapeutische Entscheidung mit einfließen lassen sollte. Die individuelle Erfahrung des Operationsteams und die Ausdehnung der malignen Erkrankung legt die Grenzen des laparoskopisch Machbaren fest. Nur durch eine Fortführung der Forschungsaktivität, der Entwicklung „intelligenter“ Instrumente sowie der Etablierung standardisierter Techniken kann die laparoskopische Chirurgie einen klaren Stellenwert bei der Behandlung maligner gastrointestinaler Tumoren erhalten. Summary. As the importance of laparoscopic surgery for benign diseases of the gastrointestinal tract continues to grow, the application of this approach in cases of malignancy remains controversial. Although the concept of cancer recurrence in the area of surgical wounds is not new, the incidence of port site recurrence is the most obvious concern. Indications and contraindications for surgery as well as a standardized nomenclature describing the type of laparoscopic procedures being performed are some other issues that need to be clarified. Complete laparoscopic procedures or the combination of laparoscopy with open techniques can offer advantages and disadvantages that surgeons will have to take into consideration when making decisions. The skill of the operating team and the extent of disease define the boundaries of laparoscopic surgery possible. The continued research as well as development of intelligent instruments and standardized techniques might give laparoscopy a clear role in the treatment of abdominal malignancies.
