Enrollment To enroll in the NextGen Patient Portal, you must request an enrollment token in person at Selma. You will be required to sign a consent form and provide an e-mail address for use with NextGen Patient Portal. Once you have an enrollment token, you can click the Enroll Now button to the right to start the enrollment process.
