A Little History… Not so long ago….

• Alaskans with disabilities were sent to Washington or Oregon to receive services – territorial days • Harborview Developmental Center was built with federal disaster funds after Good Friday earthquake allowing Alaskans to return • Categorically funded community services began to develop on road system – 4-10 bed group homes, sheltered workshops, etc. • Individualized services and funding 1990s • Closure of HDC – 1995 • Home and Community Based Waivers “Don’t take it, it’s the Devil’s money!”

CMMS Regulation Changes • Emphasis not just on quality of services, but on Quality of Life for waiver recipients;

• Definition of home and community-based settings in which services may or not be provided;

• “Conflict-free care coordination services;

• Added protections for residents in providercontrolled settings

CMMS Setting Regulations • • •



Effective March 17, 2014 Purpose: “To ensure that individuals receiving long-term services and supports through home and community based service (HCBS) programs … have full access to benefits of community living and the opportunity to receive services in the most integrated setting appropriate” “To enhance the quality of HCBS and provide protections to participants”

HCBS Settings • Integrated in and support full access to the greater community to the same degree as individuals not receiving services: – Seek employment and work in competitive integrated settings – Engage in community life – Control personal resources – Receive services in the community • Selected by the recipient from among setting options including non-disability specific settings

CMMS Regulation Changes PERSON CENTERED PLANNING • Separation of service planning and provision; • Documentation of options offered to the recipient; • Real choice for recipient free from pressure and undue influence Most of this is NOT new, but enforcing the expectations is.

Person Centered Planning A wide variety of tools and processes have been developed to support Person Centered Planning since the 1980’s. • • • • • •

Person Centered Planning Common Features: • The planning is centered on the FOCUS Person; • Focus Person identifies the Team • The plan explores the Focus person’s story • Asks to describe a desired future and compare to here and now; - Judith Snow • Identifies steps to move closer to the desired future • Relies on ongoing Team participation and support

Person Centered Planning • The PCP is NOT the Waiver POC • The Waiver POC Team is not necessarily the PCP team • The PCP does not happen one time annually and then doesn’t get revisited until the Anniversary or Renewal Date • Themes from the PCP should help define all of the services and resources needed to move closer to the dream - Paid, unpaid, Medicaid funded, and natural supports

Is It Person Centered? • Group Home • Apartment with live in staff • I own my home and have staff of my choice for support when I need it • Day Habilitation • A group in a van goes to watch airplanes at the airport • Go to walk in the mall with seniors before the mall opens • Mobile janitorial crew • Self-Employment • Self-Directed Services (micro-board)

Facilitator Training and Certification • Five 3 hour training class sessions (available by video conference) Foundations, values, pioneers & key contributors Overview of commonly used planning processes & tools Demonstration of actual plan facilitation Each student must identify a focus person and complete a PCP that meets established criterion in order to complete the class and earn certification – Students complete and submit their portfolio for evaluation – Upon successful completion, receive UAA CHD certification – – – –

Facilitator Training and Certification • CHD Person Centered Thinking, Planning & Thinking – Why should I do this? – Supported by SDS and MHTA in an effort to meet CMMS HCBS requirements – CHD Person Centered Facebook page - A place for facilitators to develop a community of support for Person Centered Thinking Planning and Services – Certified Facilitators will have the opportunity to assist with future trainings, review new plans submitted by trainees, mentor new trainees as a mechanism to maintain credentialing.
