Behavior-Based Safety Process Behavior-Based Safety didasarkan pada empat komponen utama: Pengamatan perilaku (behavioral obsevation) dan proses umpan balik; Tinjauan formal data observasi; Peningkatan tujuan, Penguatan untuk perbaikan dan pencapaian tujuan. Behavioral Observation and Feedback Ini adalah salah satu komponen yang paling penting dari proses. Pengamatan memberikan langsung, informasi yang dapat diukur pada karyawan praktek bekerja yang aman . Karyawan diamati saat melakukan tugas rutin mereka. Pengamat mendokumentasikan baik perilaku aman dan tidak aman. Karyawan tersebut kemudian diberikan umpan balik positif pada perilaku aman dan umpan balik “non-threatening” terhadap perilaku yang tidak aman. Mereka juga disediakan saran untuk mengoreksi perilaku yang tidak aman. Tinjauan Formal Data Pengamatan (Observasi) Data tersebut kemudian dianalisis untuk menentukan peningkatan karyawan (atau departemen) dalam perilaku yang aman. Hal ini dapat dilihat sebagai persentase keseluruhan. Contoh: Jika ada 20 item pada checklist dan pekerjaan dilakukan 17 diantaranya dengan aman, maka ia akan mendapatkan skor 85% safe. Peningkatan antara pengamatan dapat digambarkan dan ditampilkan pada karyawan untuk dilihat. Ketika grafik menunjukkan peningkatan, hal ini menjadi umpan balik penguat positif terhadap karyawan. Peningkatan Tujuan Menetapkan tujuan perbaikan meningkatkan efektivitas umpan balik dan keberhasilan proses perilaku berbasis keselamatan. Tujuan ini dapat mengambil form yang berbeda, seperti: 1. Persentase tujuan keselamatan 2. Tujuan Proses 3. Implementasi tujuan Penguatan dan Peningkatan Pencapaian Tujuan Manajemen harus memberikan langsung, umpan balik positif untuk memperkuat perilaku yang aman. Hadiah bisa menjadi cara yang efektif untuk memperkuat pencapaian tujuan. Hasilnya adalah: Peningkatan efisiensi Peningkatan produktivitas Peningkatan moral Peningkatkan profitabilitas

Potensi Bahaya Listrik Arus listrik antara 15 - 30 mA sudah dapat mengakibatkan kernatian, karena sudah tidak mungkin lagi untuk rnelepaskan peqanqan, Pengaruh-pengaruh lain dari arus listrik yang mengalir melalui tubuh rnanusia ialah panas yang dltirnbulkan dalam tubuh, dan pengaruh elektrokirnla. Tegangan yang dapat dianggap aman juga ada kaitannya dengan tahanan kulit manusia. Untuk kulit yang kering tahanan ini berkisar antara 100 – 500 kilo Ohm. Tetapi kulit yang basah, misalnya karena keringat dapet memiliki tahanan sampai serendah 1 Kohm. Juga luas permukaan kulit yang menyentuh ikut mempengaruhi. Akibat sentuh iangsung maupun sentuh tidak langsung dapat mengakibatkan kecelakaan serta kerugian. Kecelakaan akibat listrik dapat mengakibatkan : Kecelakaan pada rnenusia. Arus listrik antara 15 - 30 mA sudah dapat mengakibatkan kematian. Pengaruh-pengaruh lain dari arus Iistrik yang mengalir melaiui tubuh ialah panas yang ditimbulkan dalam tubuh dan pengaruh elektrokimia. Tegangan yang dianggap aman juga ada kaitannya dengan tahanan kulit manusia. Untuk kulit kering, tahanan ini berkisar antara 100 – 500 Kohm. Tetapi kulit basah, rnisalnya karena keringat dapat rnerniliki tahan sampai serendah 1 Kohm. Juga luas permukaan yang rnenyenluh ikut mempengaruhi. Kalau benda bertegangan dipegang penuh dengan tangan, pada arus kurang lebih 10 rnA saja sudah akan sulit sekali untuk melepaskannya. Kerusakan instalasi serta perlengkapannya. Jaringan instalasi listrik harus diarnankan dengan balk sesuai ketentuan yang berlaku. Gangguan Ilstrik akan dapat mengakibatkan : - Kerusakan instalasi beserta perlengkapannya (kabel terbakar, panel terbakar, kerusakan isolasi, kerusakan peralatan). - Terjadinya kebakaran bangunan beserta isinya. Kerugian. Kerugian akibat kecelakaan listrik dapat berupa : Kerugian materi (dalarn rupiah) akibat rusaknya instalasi, bangunan beserta islnya.Terhentinya proses produksi. Mengurangi kenyamanan, rnisalnya larnpu padam, AC mati, suplai air terganggu dan lain-lain. Mengingat potensi bahaya yang dapat mengakibatkan kerugian besar tersebut, sangat perlu diperhatikan prosedur keselamatan pada instalasi atau pekerjaan yang berhubungan dengan kelistrikan. H2S terdapat didalam larutan pada beberapa ini akan bertambah apabila dibaregi dengan kenaikan temperatur pada saat pengolahan (refinery). Dapat juga diketahui bahwa kontak langsung akan aktivitas dimana mungkin terjadi potensi kontak terbuka dengan gas H2S adalah : 1. Operasi pengeboran : Sirkulasi Lumpur pengeboran, bagian air yang berasal dari air dan minyak yang mengandung asam, semburan liar (jarang) 2. Pengukuan Tangki : Membuka tutup tangki pada bagian atas tangki mengalir keluar. Termasuk juga tangki-tangki lain untuk penimbun sementara maupun tangki hasil akhir (white product) 3. Perawatan Sumur (mengganti packing, mencabut tubing maupun pompa) 4. Memasuki ruang tertutup missalnya terowongan, saluran, tangki proses maupun tangki-tangki penimbunan. 5. Kebocoran pompa atau pipa (menyebabkan korosi dan mudah pecah / hancur) terhadap perawatan 6. Memisahkan belerang saat dilakukan disulfurisasi pada minyak yang berkadar asam dan tempat yang terkontaminasi oleh belerang yang mendidih 7. Penginjeksian gas yang mengandung asam kedalam formasi 8. Penimbunan aspal dan pengoperasian terkait 9. Pembersihan dengan menggunakan acid didalam sumur ataupun pada peralatan proses Kejadian Geologi H2S tidak hanya terdapat pada industri minyak di Amerika saja, akan tetapi juga pada semua industri diseluruh dunia. Hydrogen Sulfide / H2S (5), Bahaya H2S PENGETAHUAN TERHADAP BAHAYA H2S Bekerja dengan H2S atau berada didaerah yang mungkin tercemar H2S memerlukan perencanaan yang tepat untuk menentukan perlindungan secara maksimal bagi kesehatan manusia. Perencanaan tersebut harus meliputi hal-hal seperti berikut : 1. Lakukan kontak langsung dengan badan pemerintah maupun masyarakat umum. 2. Buat rencana penyelamatan (Contigency Plan). 3. TINDAKAN KESELAMATAN

1.

Bila akan mendekati/memasuki tempat kerja harus dilakukan beberapa tindakan agar dapat masuk dengan selamat :



a. Periksa tanda-tanda peringatan dan tanda bahaya audio maupun visual. b. Periksa arah angin. c. Perhatikan pekerja lain serta aktivitasnya. d. Masuklah ketempat kerja secara hati-hati.

2.

Minimal ditentukan 2 pintu keluar untuk melarikan diri (emergency exit).

3.

Memeriksa kondisi udara secara terus menerus atau menggunakan alat-alat deteksi selain sitim peringatan audio / visual yang ada. Alat deteksi portable dan pengukuran gas secara pembakaran (combustable gas meter) memberikan tingkat keselamatan tinggi.

4.

Bahaya kebakaran gas harus dikurangi dan tanda dilarang merokok harus betul-betul ditaati.

5.

Menggunakan ventilator yang kedap gas akan mengurangi resiko pencemaran karena H2S.

6.

Pelatihan terhadap bahaya H2S serta penekanan latihan dilapangan adalah suatu keharusan, pengalaman dalam laihan akan membuat kesempurnaan.

7.

Pengawasan terhadap arah angin secara terus menerus dengan menggunakan indikator angin dan wind sock adalah sangat perlu. Bila udara tercemar oleh H2S berpindahlah ke atas angin atau samping dan bukan dibawah angin.

H2S lebih berat dari udara, maka dari itu hindari berada dibagian bawah

9.

Pilih teman kerja yang sepaham ”Buddy System” untuk meningkatkan keselamatan

10. Tingkatkan dan periksa tanda-tanda peringatan. 11. Pasang tanda emergency pada tempat yang jelas dan tingkatkan sistem komunikasi. 12. Tempatkan pos gawat darurat minimal 200 ft dari sumber H2S atau di daerah yang aman. Selama gawat darurat semua personel akan berkumpul di pos ini untuk mendapatkan instruksi dan juga mengisi botol udara. 13. Konsentrasi H2S yang tinggi dapat dibuang melalui flare stack dengan dibakar. Peringgatan = Pembakaran H2S akan menghasilkan gas H2S. Akan tetapi karena udara menjadi lebih panas maka ia akan bergerak atas. 14. Pahamilah prosedur gawat darurat perusahaan terutama gawat darurat H2S yang sedikit berisi :

A. Jangan gugup B. Tahan napas C. Gunakan alat bantu pernapasan. Jangan mencoba menolong seseorang sebelum mengunakan alat bantu pernapasan (Breathing apparatus). D. Bantu mereka yang kurang menger

Hydrogen Sulfide / H2S (4), Sifat Dan Reaksi Gas H2S adalah sangat tidak stabil dan mudah terbakar, beracun dan secara alami menyebabkan karat. Terdiri dari 2 atom Hidrogen, 1 atom Sulfida. Secara biologi terbentuk karena benda organik yang mati. Mempunyai berat jenis 1,152 dibandingakan dengan udara = 1 pada temperatur 60o F. Berat ini membuat gas H2S berada dibagian bawah seperti pada selokan, ruang bagian atau tempat lain yang ventilasinya kurang baik. Pada temperatur biasa, ia dalam kondisi gas akan pada temperatur -61, 80o C berubah menjadi benda padat. H2S pada temperatur biasa dan tekanan atmosfir tidak berwarna dalam bentuk gas. Pada konsentrasi rendah H2S menimbulkan bau seperti telur busuk akan tetapi pada konsentrasi yang menggunakan indera cium akan mematikan dan tidak boleh digunakan sebagai alat ukur. H2S akan bercampur dengan berbagai cairan dan akan keluar dari larutan pada temperatur biasa. H2S adalah mudah terbakar dengan konsentrasi 43 – 45 % udara, sedang Methan hanya terbakar bila ada campuan 5 – 15 % udara. H2S akan temperatur 500o F (250o C). Bila terbakar, ia akan menghasilkan gas yang sama bahayanya yang dikenal dengan Sullfur Dioksa (SO2) H2S yang murni akan terbakar dengan nyala biru terang. H2S secara alam terdapat pada batu bara, gas alam, gas dari batu vulkanis, sulfida danau atau telaga. Dengan berbagai sifat didalam suatu perkerjaan, H2S harus dipandang sebagai sesuatu yang sangat berbahaya. Perencanaan harus baik yang pada keseluruhannya adalah menyangkut keselamatan.

Hydrogen Sulfide / H2S (3), Emergency Procedure PROSEDUR KEADAAN DARURAT (EMERGENCY PROCEDURE) Tiap tiap rig harus mempunyai prosedur standar untuk diikuti jika muncul gas H2S, dan crew harus mengenal dengan baik terhadap prosedur ini. Langkah langkah yang dianjurkan adalah: 1. Menuju ke daerah yang telah ditentukan 2. Mengambil alat pernafasan 3. Menunggu instruksi lebih lanjut Sistim buddy adalah penting untuk menjamin bahwa semua orang diperhitungkan daam keadaan darurat. Jika terjadi pelepasan H2S, jangan panik, ambil alat pernafasan, dan memakainya, membantu seseorang yang mengalami kesukaran dan kemudian menuju tempat berkumpul yang telah ditentukan. Jika sesorang terpapar H2S. Prosedur penyelamatan segera dimulai, pelindung alat pernafasan harus dipakai. Korban harus segera dipindahkan ketempat dimana tersdapat udara segar. Jika mereka tidak bernafas, penafasan bantuan dilakukan. Supervisor segera diberi tahu bahwa seseorang mengalami cedera karena H2S, sehingga penggaturan untuk pengiriman korban kerumah sakit segera dilakukan. Nomor telepon darurat atau prosedur hubungan radio diaktifkan, sehingga dapat dipakai sewaktu waktu dibutuhkan. Sistim penggunaan bendera wajib digunakan untuk peringatan tentang keadaan yang ada pada lokasi pengeboran. Bendera dan tanda wajib dipasang pada jalan masuk ke lokasi. Bendera kuning menunjukan keadaan darurat terdapatnya gas H2S. Bendera orange menunjukan adanya gas H2S dalam jumlah sedang. Bendera ini dipasang jika pengeboran telah mencapai kedalaman 1000 Feet diatas daerah bahaya yang diduga mengandung H2S. Bendera merah menunjukan bahaya yang serius. Bendera ini dipasang jika didapatinya gas H2S dalam jumlah yang membahayakan. Bendera hijau dikibarkan bila rig dalam keadaan operasi dan belum ada bahaya gas H2S. Pekerja yang telah mengikuti pelatihan penanggulangan bahay gas H2S akan melakukan suatu tindakan yang tepat bila terjadi keadaan darurat. Mereka tidak panik dalam menghadapi situasi tersebut, dibandingkan dengan mereka yang belum pernah mendapatkan pelatihan penanggulangan bahaya gas H2S.

Hydrogen Sulfide / H2S (2) PERALATAN PERNAFASAN Banyak merk dan type dari peralatan pernafasan untuk melindungi terhadap H2S. Alat ini dibagi dalam 3 kategori, yaitu : 1. Escape Unit Dipakai oleh sesorang untuk melarikan diri pada waktu emrgency 2. Work Unit Alat ini memungkinkan pekerja melanjutkan pekerjaan dengan selamat dalam lingkungan yang mengandung H2S. 3. Rescue Unit Alat pernafasan yang mempunyai suplay udara yang terletak di dalam botol. Biasanya botol ini diletakan pada punggung. Rata-rata udara dalam botol dapat mensuplai udara selam 30 menit, tetapi ini akan berfariasi dengan jumlah pekerjaan yang dilakuan dan cara bernafas.

PERTIMBANGAN TERHADAP SESEORANG Beberapa pertimbangan fisik yang perlu diperhatikan dalam penggunaan alat pernafasan (Breathing Apparatus) misalnya rambut panjang, jambang atau kumis, pamakai kaca mata yang dapat mengganggu penutup muka. Peralatan perlu di sesuaikan kepada pemakai khusus. Jika seseorang memakai gigi palsu, jangan melepas gigi palsunya, karena masker ini sudah dicocokan terhadap sipemakai untuk tekanan positif dan jika gigi palsu dilepas, akan terjadi perubahan permukaan, sehingga masker menjadi tidak cocok. Seorang dengan gendang telinga berlubang (perforated eardrum) tidak diperkenankan bekerja ditempat dimana terdapat H2S karena H2S dapat masuk ke paru paru waktu mencapai permukaan dan hal ini dipercepat dengan adanya pemanasan, melewati telinga. Pertimbangan lain juga terhadap mereka yang punya masalah tekanan darah tinggi (Hypertension) dan pernafasan. Toolpusher harus waspada keadaan fisik yang kurang mendukung dari anak buahnya. HYDROGEN SULFIDA / H2S (1) PENGETAHUAN GAS H2S Hydrogen Sulfida yang juga dikenal sebagai gas H2S, gas asam, gas telur busuk adalah gas yang dapat mematikan dan ditemui pada waktu pengeboran minyak. Jika suatu sumur dibor, dan masuk kedalam formasi yang mengandung H2S, gas ini dapat tersirkulasi ke permukaan dalam lumpur bor atau fluida lainnya yang disirkulasi seperti gas, minyak atau air. Beberapa tempat di RIG mempunyai potensi bahaya dengan kebenaran H2S, antara lain ”mud return line” bell niple, siple, shale shaker, penampungan lumpur bor yang berlokasi rendah atau selokan, tangki lumpur atau bagian lain dari pada sistim sirkulasi termasuk pompa lumpur, pipa lumpur, dimana kemungkinan terjadi kebocoran. SIFAT H2S Karena H2S lebih berat dari pada udara, maka gas ini akan mengumpul dilokasi yang rendah, terutama jika tidak ada, sehingga perhatian perlu diberikan kepada lokasi lokasi yang rendah dalam rig ini. Gas ini tidak berwarna, akan terbakar atau meledak didalam udara. Jika gas ini terbakar akan memberikan bahaya yang lain, karena terbentuknya gas SO2 dalam jumlah yang kecil, gas ini berbau seperti telur busuk, tetapi deteksi adanya H2S berdasarkan bau ini tidak dapat dijadikan pegangan, karena H2S dapat mematikan saraf penciuman. Dampak H2S tergantung pada berapa lama terpapar (exposure) dan konsentrasi dari gas tersebut. Dosis H2S yang besar dapat mengakibatkan seseorang tidak sadarkan diri dengan segera. Jika korban tidak segera di pindah dari daerah tersebut, dan di tolong dengan pernafasan buatan (artificial resuscitation) maka korban dapat mati atau mengalami kerusakan otak. Seseorang yang menghirup H2S dengan dosis rendah dalam waktu 3 sampai 15 menit, dapat menyebabkan mata berair, kulit mengalami iritasi dan batuk. TINDAKAN PENCEGAHAN Ada beberapa cara untuk melindungi orang orang di rig dari keracunan H2S. Karena angin yang berhembus dapat dengan mudah membuang H2S, maka orang orang tersebut akan segera bebas dari ancaman keracunan H2S. Kalau memungkinkan orang orang tersebut berpindah kearah lokasi yang tinggi, karena sifat H2S akan mengumpul didaerah yang rendah.seseorang jangan berpindah menuju Well Head, meskipun pada posisi Upwind, karena well head merupakan sumber dari pada H2S. Windsock atau pita panjang (streamer) dapat menunjukan kemana angin berhembus. Bug Blowers dapat dipakai untuk mengusir H2S jika udara tenang. Agar orang orang (Crew) rig mengetahui apa yang harus dilakukan dalam meghadapi H2S, briefing perlu di berikan terhadap crew yang bekerja dalam daerah yang mungkin mengandung H2S. Suatu daerah aman wajib ditentukan sebagai tempat daerah berkumpul dimana mereka akan mendapat penjelaan atau instruksi pada waktu terjadi emergency (keadaan darurat). Perlu di tentukan dua tempat, sehingga kemana pun angin bertiup, salah satu akan menjadi upwind. Lumpur bor perlu cukup berat untuk menghadapi kecelakaan H2S yang terjadi, juga penetralisasi H2S perlu diberikan kedalam lumpur untuk menjaga gas mencapai permukaan. Detector dapat di tempatkan pada shale shaker, bell nipple, tangki lumpur dan tempat tempat lain dimana H2S mungkin muncul.
