Blood Count A complete blood count (CBC) is an important diagnostic test that provides information about the type and number of blood cells contained in a patient sample. The sample is usually treated with EDTA to prevent clotting. Qualitative or quantitative aberrances in the blood cells are indicators of a vast number of medical conditions like anaemia, leukaemia or infections. There are three types of blood cells: red blood cells (erythrocytes), white blood cells (leucocytes) and platelets (thrombocytes). Red blood cells constitute about 95% of all blood cells. The percentage of red blood cells in the total blood volume has been termed “hematocrit” (HCT or Ht) and is normally around 40% in women and around 45% in men. The complete blood count can be ordered with or without differential (dt.: großes bzw. kleines Blutbild). A complete blood count without differential gives information about red blood cells and platelets but only gives the total number of white blood cells. The differential provides detailed information about the different types of white blood cells.

Red Blood cells (RBC) Red blood cells are the most common of all blood cells. They are responsible for delivering the oxygen from the lung to the rest of the body. Normal red blood cells are disc-shaped with indentations on both sides. Mature red blood cells do not possess a nucleus or other cell organelles and consist mainly of hemoglobin, a protein containing iron atoms which are responsible for the red colour. Hemoglobin can temporarily bind oxygen and will change its colour depending on the presence or absence of oxygen. It is scarlet when bound to oxygen and dark red without. White Blood cells (WBC) White blood cells are part of the immune system. There are five types of white blood cells: neutrophil, basophil and eosinophil granulocytes, and two types of agranulocytes, namely lymphocytes and monocytes. Neutrophils are the most common white blood cells that target and destroy bacteria, viruses and fungi. Lymphocytes, the second most common white blood cells, can be further divided into B-cells, Tcells and natural killer cells (NK-cells). These cells identify and attack “invaders” (e.g. bacteria, parasites), tumours and virus-infected cells. Monocytes migrate to other tissues where they differentiate into Macrophages (function: disposal of cellular debris and pathogens) or dendritic cells (function: “showcasing” foreign material to activate T-cells). Eosinophils mainly target multicellular parasites and play a role in allergies and asthma. Basophils release histamine and also play a role in asthma and allergies. Platelets (PLT) Platelets are the smallest of all blood cells. Like red blood cells they do not contain a nucleus. They play an important role in blood clotting. Damage to a blood vessel activates the platelets that aggregate to form a plug that is then further strengthened by fibrin. Aggregated platelets secrete substances that promote the repair of the damaged tissue. Normally, platelets are disc-shaped but will change to a stellate shape when they are activated.

In general, plasma treated with anticoagulants that bind calcium (mainly EDTA) is used for blood counts. A blood count can be done manually or by an analyzer. For a manual blood count a defined volume of diluted blood is put into a counting chamber and the blood cells are counted under a microscope. For a differential a thin layer of blood is smeared on a microscope slide and then stained. The percentage of each cell type is determined for a large number (usually 100) of white blood cells. Manual blood counts are especially useful for patients with abnormal blood cells that an automated analyzer might not identify reliably. Automated blood counts are performed by special analyzers. Blood flows through a thin tube and passes detectors that identify and count the cells (flow cytometry). It is vital that the cells pass the detectors one at a time. This is usually achieved by hydrodynamic focussing, where the blood is injected into a stream of flowing liquid, e.g. saline solution (see figure). The different types of cells can be identified visually and/or by measuring their impedance.

Flow cytometry Laser light scatter technique For visual identification and counting a laser beam is used. Cells passing through the laser beam will deflect the light. Detectors then measure the amount of remaining light or the amount of scattered light. Forward scatter: light that is only minimally deflected. The size of the cells determines the amount of forward scatter. Often, the light of the laser beam itself is obscured so that only scattered light can reach the detector. Side scatter: light that is deflected to the side. Complex structures inside the cell cause side scatter. A Lens system focuses the scattered light onto a detector usually placed at a right angle to the laser beam. Different cell types can be distinguished by the amounts of forward and side scatter, e.g. Lymphocytes (small size, low complexity = little forward scatter, little side scatter); Monocytes (medium size, medium complexity = average forward scatter, average side scatter); Granulocytes (large size, high complexity = much forward scatter, much side scatter).

Aperture impedance technique The impedance method (also known as Coulter method) has been used for automated cell counts since the 1950s. The detector consists of two measuring chambers connected only over a small aperture that contain a solution with suspended cells. Two electrodes placed on either side of the aperture generate a static electrical field. The voltage measured between those two electrodes depends on the size of the aperture and the conductivity of the solution. In order to count the cells the solution has to flow through the aperture from one chamber into the other. The impedance (or electrical resistance) of the cells is much higher than that of the surrounding fluid. Accordingly, cells passing through the aperture cause a disturbance of the electrical field by basically “blocking” the electric current. The amplitude of this signal is proportional to the size of the cell. As is the case for the laser light scatter technology it is very important that the cells pass through the aperture in single file.

Fluorescence It is also possible to use fluorescence to directly mark certain cell types. The cells are labelled with specific antibodies coupled with fluorescent material. When this fluorescent material is “activated” (usually by exposure to light) it will emit light of a specific wavelength (aka colour). By filtering the emitted light multiple labels emitting light of different wavelengths can be measured simultaneously.

Light absorbance For direct measurement of hemoglobin the red blood cells are destroyed so that the hemoglobin is released. Then the intensity of light of a specific wavelength that has passed through the solution is measured.

Red Blood Cells An increase in red blood cells is termed polycythaemia (also known as erythrocytosis), a decrease is called anaemia. Anaemia is the most common blood disorder. Different forms can be classified according to the size of the red blood cells (micro-, normo- or macrocytic) and the amount of hemoglobin in each cell (hypo-, normo- or hyperchromic). Qualitative or quantitative changes in the red blood cells can be due to a variety of causes. For differential diagnosis a number of parameters are determined. The plausibility of the values for the three main parameters can easily be checked with the following rule: RBC x 3 = HGB HGB x 3 = HCT RBC (Red Blood Cells) Number of red blood cells, given in cells per picolitre. Normal range: 3.8-5.2 cells/pl (women), 4.4-5.9 cells/pl (men) Low numbers of red blood cells can be due to decreased production, a shortened lifespan or a destruction of the red blood cells (hemolysis), blood loss, or increased volume of blood plasma. Heightened levels of red blood cells can be caused by increased production in the bone marrow, chronically low oxygen levels (e.g. in high altitudes), or decreased plasma volume (e.g. due to dehydration). HCT (Hematocrit; also Packed Cell Volume, PCV) Percentage of red blood cells in the total blood volume. Normal range: 35-47% (women), 40-52% (men) The hematocrit can easily be determined by centrifuging blood in a capillary tube. Therefore this test can also be performed outside a clinical laboratory. Automated analyzers will calculate the HCT-value as MCV * RBC/1000 (in this case MCV is measured directly). The HCT levels of professional athletes are monitored to check for blood doping. HGB (Hemoglobin) Amount of hemoglobin, given in grams per decilitre. Normal range: 11.7-15.7 dl (women), 13.3-17.7 dl (men) HGB is measured photometrically at 540nm. agile – affordable – accurate

MCV (Mean Corpuscular Volume) Mean volume of the red blood cells, given in femtolitre. Normal range: 81-100 fl The MCV is calculated as HCT[1/1]*1000/RBC when the blood count is done manually. Automated analyzers size the red blood cells and determine the MCV from the histogram. MCH (Mean Corpuscular Hemoglobin) Mean amount of hemoglobin in a red blood cell, given in picogram. Normal range: 26-34 pg (women), 27-34 pg (men) Calculated as HGB*10/RBC MCHC (Mean Corpuscular Hemoglobin Concentration) Mean hemoglobin concentration in a red blood cell, given in percent. Normal range: 31.4-35.8% (women), 31.5-36.3% (men) Calculated as HGB*100/HCT RDW (Red Blood Cell Distribution Width) Variation in size of the red blood cells. Normal range: 11-15% Values lower than the normal range are not clinically significant. Heightened values are found in certain forms of anaemia (e.g. iron deficiency anaemia). The RDW is calculated as: (Standard deviation of MCV*100)/MCV RDW_CV (Red Blood Cell Distribution Width – Coefficient Variation) is calculated using the width of the MCV distribution curve at 60% of peak. RDW_SD (Red Blood Cell Distribution Width – Standard Deviation) is calculated using the width of the MCV distribution curve at 20% of peak.

Platelets PLT (Platelets) Number of platelets, given in cells per nanolitre. Normal range: 150-450 cells/nl Low platelet levels (thrombocytopenia) can be due to problems with the production of platelets (e.g. in sepsis or some forms of leukaemia), increased consumption or destruction, or a side-effect of certain drugs (e.g. chemotherapy drugs like interferon). Thrombocytopenia can lead to bruising or bleeding. EDTA (the anticoagulant routinely used for blood counts) can induce a pseudothrombocytopenia that can be detected by a second platelet count using citrate instead of EDTA. An increased platelet count (thrombocytosis) can be reactive (e.g. after blood loss, in inflammation or in iron deficiency anaemia) or essential/primary (in myeloproliferative diseases, when the bone marrow produces an excess number of cells). Reactive thrombocytosis is transient and usually does not carry an increased risk of thrombosis. Essential thrombocytosis can lead to thrombosis in some patients. MPV (Mean Platelet Volume) Mean volume of the platelets, given in femtolitre. Normal range: 7.5-11.5 fl When platelets are produced in increased numbers they are larger than normal while they are smaller when produced in small numbers. Thus, MPV size gives information about platelet production in the bone marrow. PCT (Plateletcrit/Thrombocrit) Percentage of platelets in the total blood volume. Normal range: 0.1-0.5% Calculated as PLT * MPV * 100. PDW (Platelet Distribution Width) Variation in size of the platelets. Normal range: 12-16% Values higher than the normal range indicate increased destruction of platelets. agile – affordable – accurate

White Blood Cells/Differential WBC (White Blood cells) Number of white blood cells, given in cells per nanolitre. Normal range: 4.3-10 cells/nl Increased numbers of white blood cells (leukocytosis) is commonly found in connection with acute infections (bacterial, viral, fungal, parasitic), inflammation, allergies, cancer (primarily leukaemia), poisoning, and as a side effect of certain medications (e.g. corticosteroids). Stress, physical exercise or smoking can also induce a modest increase in leukocyte numbers. For differential diagnosis it has to be determined which type of white blood cell is primarily increased in number. Most commonly, leukocytosis is due to an increase in the number of neutrophils. A decrease (leukopenia) in the number of white blood cells increases the risk of infection for the patient. It is a commonly found after chemotherapy, in some types of cancer or infections (e.g. HIV), in aplastic anaemia, as a side effect of treatment with certain drugs, but also as a result of physical or mental stress. NEU# (Neutrophils, total number) NEU% (Neutrophils, percentage) Number of neutrophils, given in cells per nanolitre or in percentage of all white blood cells. Normal range: 2.5-7.5 cells/nl 40-75% of WBC An increase in the number of neurophils (neurophilia) is generally associated with bacterial infection or tissue necrosis (e.g. heart attack, burns). LYM # (Lymphocytes, total number) LYM% (Lymphocytes, percentage) Number of lymphocytes, given in cells per nanolitre or in percentage of all white blood cells. Normal range: 1.5-3.5 cells/nl 20-45% of WBC Heightened lymphocyte counts (lymphocytosis) is the result of acute or chronic infections (e.g. pertussis, tuberculosis), especially in children. Acute or chronic lymphocytic leukaemia (CLL) is also characterized by lymphocytosis. agile – affordable – accurate

MON# (Monocytes, total number) MON% (Monocytes, percentage) Number of monocytes, given in cells per nanolitre or in percentage of all white blood cells. Normal range: 0.2-0.8 cells/nl 2-8% of WBC Increased monocyte numbers (monocytosis) can often be found in the recovery phase of an acute infection, but also in chronic infection (e.g. tuberculosis, malaria) and in certain types of cancer (e.g. Hodgkin disease, chronic myelomonocytic leukaemia CML). EOS# (Eosinophils, total number) EOS% (Eosinophils, percentage) Number of eosinophils, given in cells per nanolitre or in percentage of all white blood cells. Normal range: 0.04-0.4 cells/nl 1-6 % of WBC The most common causes for an increase in the number of eosinophils (eosinophilia) are allergies (e.g. hay fever, asthma) or parasitic infections. It can also be elicited by in certain forms of cancer or infections. BAS# (Basophils, total number) BAS% (Basophils, percentage) Number of basophils, given in cells per nanolitre or in percentage of all white blood cells. Normal range: 0-0.1 cells/nl 0-1% of WBC An abnormal elevation of the basophil number (basophilia) is rare. It is found in myeloproliferative diseases, especially in CML, and can also be found in patients with certain infections or with severe thyroid hyperfunction.
