March 2013 Merlin conducted a needs assessment in Dobley, following a huge turnover influx of returnees into Somalia. This assessment revealed that there were only two health facilities in Dobley, one supported by WRRS (LNGO) and the second health facility was Dobley hospital, which is a community hospital serving an estimated total population of 50,000 (UNHCR). As far as Cemonc services are concerned there is no facility that could provide Cemonc services over a stretch of 310 Kms right from the outskirts of Kismayo to Dadaab refugee hospital.Even Bemonc services was are not available this was mainly due to the lack of funding, and shortage of qualified human resources. There was no existing referral system in place. Patients requiring caesarean sections and with other obstetric complications were going to Daadab refugee camp in Kenya which is 110 km from Dobley on bad roads and cross border travel document were required of which many people do not have thus hindering referals. There is no health facility in the entire stretch who which can provide in-patient services to manage the life threatening emergency cases. Records from the two facilities revealed that children under the age of five years suffer mainly from Respiratory Tract Infections (RTIs), Diarrhoea, Malaria and Skin Infections, while adult morbidity include non-communicable diseases like hypertension, diabetes and Arthritis. Communicable disease surveillance was not monitored by any of the health partners in Dhobley. IOM has now mobile clinic team who goes around to villages. The local authorities approached Merlin to provide assistance in terms of health service provision in terms of 24/7 Bemonc and Cemonc services along with OPD services. SCI had provided 24657 Consulations, In children < 5years girls 4243, boys 4367 , in adults women 8155 and men 7892 consultations and has conducted 337 deliveries assisted by skilled birth attendants during the past 12 months. In order to provide C/sections there was a need of laboratory and bloodbank for Cemonc services and SCI has raised some funds and has established a lab and a blood bank, Cemonc services is now fully functional. But the CHF funding is coming to an end by mid May 2015 thus SCI seeks to continue to provide Bemonc, Cemoc, OPD services in Dobley hospital after May 2015, to introduce inpatient services from March 2015,and to undertake Ultrasound scanning of women to diagnose different conditions like Intrauterine death of the foetus, fetal presentation, position and status of placenta in APH, assessment of polyhydramnious and oligohyramnious, detection of fetal anormalies, confirmation of incompleteness of abortion, detection of molar pregnancy etc.The nearest place for ultrasound is Dadaab refugee camp. These new activities needs 2 nurses, 3 auxiliary nurses and medical supplies for 1 year. 24/7 provision of In-patient services in Dobley hospital for life threatening emergency cases only. A strict admission criteria will be developed and followed to admit only the acute medical cases cases in Dobley hospital All admitted patients are managed under the supervision of a Medical doctor. Standard nursing care will be provided to all in-patients. The cases that cannot be managed can be referred to Dadaab refugee camps Health facilities (100kms)with an Ambulance hired by Save the children.This section is about about needs assessment whereas the red font above is abaout justification so I am not sure of its relevance here.. Activity 1.3 Obstetric complications including C/sections normal deliveries are managed using the standard treatment guidelines at Dobley hospital Indicators for outcome 1 Health At least 1 health facility with functional comprehensive emergency obstetric care (CEmoC) in Dobley 1 Indicator 1.2 Health Number of staff who underwent refresher training on Cemonc and Bemonc Indicator 1.3 Health Number of normal & complicated deliveries conducted by the skilled birth attendants at Dhobley hospital during the project period Target 94 C/sections; 187 other complications; 478 normal deliveries total=759 Outcome 2 Preventive and curative services targeting the entire population (covering all age groups and both the genders) is strengthened in Dobley Hospital Activity 2.1 Children under 5 years and women of child bearing age are completely immunized against vaccine preventable diseases Activity 2.2 Consultations and treatment is provided to all age groups (5) covering male and female patients for both communicable and non-communicable diseases in the OPD. Activity 2.3 Hire 24/7 ambulance services to bring mothers to the facility and one ambulance will be hired only on need basis when inpatient is to be trrasferred to Dadaab refugee hospital. Activity 2.4 24/7 provision of In-patient services with 15beds in Dobley hospital for life threatening cases only . A strict admission criteria is developed and followed to admit the cases in Dobley hospital All admitted patients are managed under the supervision of a Medical doctor.
