Link Hey /r/ems, I work at medical billing company in NJ and I am currently trying to put together an appropriate SOP manual. I know that medical billers get a bad rep. here, but I am actually an IT/IS student in a unique situation. It would mean a lot if you NJ EMS guys could help me out, I am really overwhelmed and confused about where to begin and what to include.

What are you even asking? SOP as in a full set of medical protocols? A "price list" for various procedures? If you want a full set of protocols, that needs to be written by or at least signed off by an MD, who is the medical director of the service they apply to. I'm just trying to figure out what your goal is and what information you're looking for. permalink

I am being forced to write the entire SOP manual for an EMS company. I need examples of what is required in the SOP manual as dictated by the NJ Dept. of Health Services. What I have been able to gather is this: NJAC 8:41-3.12 NJAC 8:40-3.5 Where and how would I be able to get this information required, if that is even possible? permalink

I'm having a hell of a time with my internet right now, but SOPCenter has some great examples on Vehicle Operations, Patient Dispostion, Special Ops, and many more. FEMA has a general guide on developing SOPs, although that's about a decade old. The Apache Junction Fire District has many examples, such as this one on restraints. Mount Dora Fire's EMS division offers this comprehensive document. As you look through these, just remember that many are from out of state and your guidelines must follow state law. For example, remember in New Jersey that on duty units have a duty to act at any safe scene in the state. Remember that we have mandatory reporting for shootings, stabbing, child and elder abuse. Remember that we have no exemptions from traffic laws for emergency vehicles operating with lights and sirens - you must drive with due regard. Also make sure that any special ops protocols fit in with ICS/NIMS. It is a common misconception that command goes to the highest ranking member of an organization present. Sorry about the double post - I'll edit this one to add one more comment: Your SOPs must be clear and consistent. Don't say 'should', say 'shall' or 'must'. Don't say 'about 25 mph', say 'not to exceed 25 mph'. Try to avoid run on or complicated sentences - you don't want indecipherable legalese, but you also don't want to sound like a 5th grader. Make it professional and make it instructive. Provide a clear process for comments and complaints both before and after implementation. Let the boots on the ground tell you what they need. If you can establish a good working relationship between yourself and the department then you'll find that you are able to be much more effective and much more persuasive. permalink

Try to get some examples from other agencies. You'll basically can specify everything from what constitutes a valid ambulance 'crew', protocols for driving, vehicle maintenance, equipment maintenance, as well as actual patient care. It can contain a lot of information if you decide to specify everything, and you should work off of an existing SOP from this agency or another to get you started. SOPs are procedures for really anything nonmedical (anything that goes step 1, step 2, etc with no exceptions - rig checks and such), SOGs are guidelines (anything that isn't a procedure, such as "a crew must contain two EMTs" or "ambulances must be refueled before reaching 1/2 tank" or "any vehicle problems are to be reported to the supervisor immediately"), and protocols are everything medical (EXAMPLE ONLY NOT MEDICAL ADVICE "nitroglycerin shall be administered to any patient complaining of chest pain consistent with a cardiac origin with a blood pressure greater than 90 mmHg systolic who has not already taken 3 doses and who has not taken a dose in the last 5 minutes if the medication is prescribed to them and not expired") - this is the part that requires a MD or DO, and the medical protocols are usually kept as minimal as possible - each medication plus spinal immobilization is really all you need at the BLS level, maybe airways. Remember that the more you write, the more options you take away from your crews, so it's important to balance giving your crews the flexibility they need while retaining the liability protection that you need. Your complete final document should be reviewed by the service's medical director and legal counsel prior to implementation. permalink

7. Only submit memes, image macros, reaction gifs, rage comics, cringe shirts, etc. on Mondays and you must write " [MEME]" in the beginning of the post's title. This rule does not apply to comment sections.

I have researched NJAC Title 8:40 and 8:41, I am just out of my element in a sense that I don't know administrative or medical procedures, nor do I know what needs to be included. permalink
