EXECUTIVE DIRECTOR Can you believe that the year is half over? It seem like we just had the Holiday Celebration and Christmas Parade and here we are making plans for the 2018 Parade. We are also preparing for the hot summer and all that takes place during that time. Things like Swimming Lessons, Summer Day Camp, Boys Basketball Camp, Ballet, Taekwondo, Art Lessons, Mentoring, Volleyball, T-Ball, Pee Wee Baseball, Soccer, Kickball and a multitude of senior activities. Not to mention all the spring and summer entertainment that comes your way. Things like Swing Into Spring Festival, Community Wide Yard Sale, BBQ and Jazz, Car and Bike Show, Family Swim Night and the list goes on and on. This brings me to you. What are you doing for yourself and your family? Are you involved in a family activity together? Do you go to the movies, concerts, or just for a walk together? Time together is very important in family development. Remember, a family that plays together stays together and builds memories. Here at the Recreation and Leisure Service Department, we want to promote and meet the needs of the community through recreation and leisure activities at an affordable range and to ensure fun and enjoyment that encourage positive community values. We are committed to earning the trust, respect, and support of our citizens. We invite you to get involved, become a leader, participate and most of all let us help you. The Recreation and Leisure Services Department reserves the right to cancel any class due to insufficient registration. If cancellation is necessary, anyone registered will be so informed and a full refund shall be automatically issued and mailed to your residence. Refunds, less 25% will be given after payment has been processed, or a class begins only after a doctor’s statement of the participant’s incapacitating illness or injury. A written request for the refund must be accompanied by the doctor’s statement. Refund request must be made prior to class ending.

our citizens, registrants and participants in Recreation/Leisure programs, special events or facilities. FACILITY RENTALS RULES AND REGULATIONS PURPOSE: The purpose of facilities is to satisfy the activity, social, meeting space and recreational needs of the citizens of Forest Park. Certain rules and regulations become necessary to assure fair usage and to provide the maximum effective use for the greatest number of people possible. 1. All activities conducted on City of Forest Park Recreation and Leisure Services property must be in compliance with local, state and federal laws and in keeping with standards of reasonable behavior. 2. Use of recreational facilities by non-recreational affiliated groups shall not have priority over, nor in any way conflict with any recreation affiliated activity. 3. The department may approve the use of recreation facilities by non-recreation groups in the following categories: Forest Park residents and businesses Civic groups and churches Commercial Associations Profit and Non-profit Schools Out of City individual, group, and business 4. Proof of residency is required in the form of a current state of GA Driver’s License, State ID Card, Military ID, or Passport. If address is not current on Driver’s License, a current utility bill or lease will be required. 5. Rentals will only be issued to persons 21 years of age or older. 6. Renter is responsible for damages which may occur during their renting of facilities and fees may be accessed for damages. 7. Specific approval by Recreation and Leisure Director or Deputy Director of Recreation is required if an event will involve admission fees or fund raising, or other charges. Nothing shall be sold, given, exhibited, or displayed without permission. No inflatable devices, bounce houses, climbing walls or other amusement devices allowed without permission. Use of tents. Use of portable grills. No amplified music. No music is to be played loud enough to disturb or be offensive to other people using the park area. A letter is required containing day(s), dates, time facility or facilities requested, equipment to be used, items to be sold, exhibited or displayed, a detailed description of function and explanation of where funds are going to be used. Letter must be received at least three (3) months prior to requested date. 8. Each user group shall agree to “hold harmless” the City of Forest Park, Recreation and Leisure Services Department for any injury or damage to any person in the use of said premises. 9. The original copy of the reservation form must be presented before the facility will be opened. 10. The person named on the rental SHALL be responsible for the function and any action or conduct of any individuals participating in the activities on the rented facilities shall be their responsibility.

11. Rental party must vacate the facility when the reservation time expires. The facility WILL BE USED FOR THE RESERVATION TIME ONLY by the renting party. Renting party must vacate the facility whether people are waiting to use the facility or not. 12. Renters must agree to clean up the facilities after the function by depositing all refuse and litter in trash containers and to include the restroom in cleanup where provided. 13. All activities must be completed by 9:00pm unless prior approval has been given by the Recreation and Leisure Services Department and is noted on the rental agreement.

14. All decorating, covering or changing of the facility and equipment should be discussed at the time the reservation is made and put in writing as a part of the rental agreement. All decorations shall be put up and taken down on the day of the function during the period of time for which the facility is reserved. The use of cellophane, scotch or adhesive tapes, nails, staples, etc. on the tables or other equipment or the facility are not allowed. 15. Groups composed of ages 11 years and under must be supervised by one adult for every ten (10) children at all times while they are using the facilities. The reservation must be taken out by one of the ADULTS WHO WILL BE SUPERVISING THE FUNCTION. 16. Groups composed of ages 12-17 years must be supervised by one adult for every 15 youths at all times while they are using the facilities. The reservation must be taken out by the ADULTS WHO ALL BE SUPERVISING THE FUNCTION. 17. Alcoholic beverages permitted on City of Forest Park, Recreation & Leisure Services property with approval permit from the Planning Building & Zoning Dept. 18. PROHIBITED (including but not limited to): Smoking, gambling, disorderly conduct, boisterous or profane language. 19. Bands or sound equipment are not allowed at outdoor facilities without written permission from the Recreation and Leisure Services Department. 20. No vehicles other than City of Forest Park vehicles will be allowed on the service road unless authorized in writing by the Recreation and Leisure Service Department. 21. ABSOLUTELY no motorcycling, bicycle riding, roller skating, skateboarding, etc. is allowed on any Recreation and Leisure Services Department’s tennis courts, walking track, pool decks, gym floor, ball fields, picnic sheds, or entertainment facility. 22. An employee of the Recreation and Leisure Services Department must be present to open and secure the building and to supervise certain activities, which call for it. The employee shall have the authority to determine the play-ability of facilities, to request assistance from the police if deemed necessary. If unruly behavior or unsportsmanlike conduct is displayed by participants, spectators, coaches, players, the employee of the Recreation and Leisure Services Department has the authority to take disciplinary action including suspension of the activity or barring the renters from future use of parks, recreation and leisure facilities. 23. THE CITY OF FOREST PARK RECREATION AND LEISURE SERVICES DEPARTMENT HAS THE RIGHT TO REVOKE A RENTAL AGREEMENT FOR VIOLATION OF ANY OF THESE POLICIES.

FACILITY RENTAL INFORMATION The Department’s facilities are available for rental. The fees listed below are established for short-term rentals by non-profit groups. For long-term rentals or profit groups a request in writing must be submitted to the Director of Recreation and Leisure. Terms, agreements, insurance, and fees will be determined. Fees for organizations outside the city limits of Forest Park, Georgia will, in most cases, be doubled. 1. ATHLETIC FIELDS All athletic baseball/softball fields - $25/ hour Resident/Department participant - $15 /hour Non-resident - $25/ hour Athletic Associations - $15/game Semi-Pro - $30/game Tournaments/League Teams Play - $25/team KIWANIS STADIUM Semi-Pro Football - $200 per game Middle School/JV/High School Teams - $100 game Special Event – Non-Profit - $150 per day Special Event – Profit - $300 per day Local Associations/Organizations - $75 to $150 per day SPECIAL CONTRACTS, AGREEMENTS, INSURANCE MAY BE REQUIRED. MAINTENANCE PERSONNEL AND FACILITY PERSONNEL MAY ALSO BE REQUIRED AT AN ADDITIONAL COST. FIELD DRAGGING AND LINING OFF IS AVAILABLE AT A RATE OF $20 EACH TIME. Sports (Baseball/Softball) fields are available for reservations and/or rent by individuals, groups, or organizations. Rental agreement must be made at least one (1) week prior to the tournament. A special contract is required for privately managed tournaments. Personnel at renter’s expense may be required. Other facilities such as concession stands are also available for rental. Call for information. 2. AQUATIC RENTAL Family Reunion, birthday parties, or picnics. Cost is dependent upon number in party. PLEASE CALL FOR MORE INFORMATION. Pools are available for rental by Private, Therapeutic, Associations or Schools. Contracts, insurance and agreements may be required. Call for more information. 3. GYMNASIUM Resident Restricted or Limited Use - $50 per hour – 2 hrs min. ~ $300 per day Resident Non-Profit Group/Org. - $100 per hour – 2 hrs min. ~ $600 per day Non-Resident - $100 per hour – 2 hrs min. + $40 each additional hour ~ $600 per day SPECIAL CONTRACTS, INSURANCE REQUIRED. STAFF WILL BE REQUIRED TO BE PRESENT THROUGHOUT THE EVENT AT A RATE TO BE DETERMINED ACCORDING TO CURRENT RATE OF PAY + A POLICE OR FIREMAN MAY ALSO BE REQUIRED.

ALL SALARIES WILL BE THE RESPONSIBILITY OF THE RENTER. Gymnasium is available for reservation and/or rent by individuals, groups, or organizations. The availability of this facility is limited and varies with the seasons. Special contracts, insurance, or staff may be required. Call for more information. 4. PAVILIONS/PICNIC SHEDS Shed – 9am-9pm - $100/ residents or $125/ nonresidents Sunday Rentals - 12 noon - 5 pm $60/ residents or $85/ non-residents 5. ENTERTAINMENT FACILITY (Cleaning/Security Deposit $250) Reservations and permits required. Resident - $150/4 hr s + staff @ $30/hr ~ $300/8 hrs + staff @ $30/hr Nonresident - $300/4 hrs + staff @ $30/hr ~ $600/8 hrs + staff @ $30/hr Special permits, agreements, or contract may be required as well as personnel. Call for information. 6. 696 MAIN STREET or 850 MAIN STREET FACILITY FEES & RENTAL REQUIREMENTS Rental Hours: 696 Main Street or 850 Main Street (Banquet Room) hours available for rental are Monday through Friday 9:30am-4pm or 5pm-9pm; Saturday 9am-10pm; Sundays 12pm-5pm. (Minimum of 4 hrs) Recreation and Leisure staff will determine the proper classification of the applicant; ascertain that the use, time, date, and size of group are appropriate and available. Reservations are not confirmed until all requirements have been met and full payment of all fees and deposit has been received. The City may cancel or deny the application of any person or group when it is determined by the City that the proposed use of the facility will not be in the best interest of the City. A copy of the approved permit will be provided to the applicant. Reservations will not be confirmed until payment is received. CLASSIFICATION OF USE The City of Forest Park Recreation & Leisure Department fee structure for 696 Main Street will be based on the residency of the user/renter. Any organized activity must get prior approval to utilize any city facility per city ordinance. Any for-profit or open to general public by any individual, organization, or group must provide one hundred thousand dollar ($100,000) insurance rider listing the City of Forest Park as additional insured. A cleaning/damage deposit of two-hundred fifty dollars ($250) will be required on all rentals. The deposit will be refunded if all facility’s rules are followed and the facility is left in good condition. If the facility is damaged or left a mess, or if the Police Department has to be called out, the deposit will be forfeited. Current Facility Rules & Regulations will be enforced. FEES FOR RENTAL OF 696 MAIN STREET or 850 MAIN STREET Reservations are accepted in four (4) hour segments. The cost of additional hours are: $15 per hour for residents and $25 per hour for non-residents.

Security Deposit of $250 is required for each facility. Facility fees are calculated on an hourly basis. Be sure to include all set-up and delivery time as part of your activity time. When calculating ending time, include anytime needed for break down and clean up. You will incur additional fees and/or staff charges if your event runs beyond the contracted hours. Security (Forest Park Police Department) is required for certain activities. Renter is responsible for contacting Forest Park Police Department and notifying the Recreation & Leisure Deputy Director upon confirmation. Security will be present for the entire event. The person making the reservation must be present the entire time listed on the contract. The person making the reservation is responsible for seeing that the area around the facility is clean and well kept during and after the event and for ensuring compliance with all City rules. The applicant is also responsible for the removal of all decorations and any items of property brought to the Facility. Trash shall be deposited in appropriate containers (located in rear of building). If there is more trash than the containers will allow, the person is also responsible for removal of extra trash from the grounds. Prior to leaving the facility, the applicant is responsible for cleaning and keeping area in order.

FOOTBALL & CHEERLEADING Register: June 2, 9, 16, & 23, 2018 12pm -4pm at Kiwanis Stadium Cost: $155 per child This program is sponsored by Forest Park Youth Football. Forest Park Youth Football and Cheerleading is a member of Georgia Youth Football Conference. This program is boys and girls ages 5-12 years of age. This is for beginners and experienced children. For more information call Michael A. Morris, Sr. at 404-3632908. FOOTBALL CAMP This camp is for boys and girls ages 5-14 years old. It’s FREE of charge. The camp will be July 7, 14, 21, & 28, 2018 10am-12noon. The camp is ran by Forest Park Youth Football coaches. The camp will offer a strong foundation of basic fundamentals, with no contact. For more information call 404-363-2908. CHEERLEADING CAMP This camp is for girls ages 5-14 years old. It’s FREE of charge. The camp will be July 7, 14, 21, & 28, 2018 10am-12noon. The camp is ran by Forest Park Youth Cheerleading coaches. The camp will offer a strong foundation of basic fundamentals.

GIRLS SOFTBALL CAMP This camp is for girls ages 8-17 years old. This camp is ran by our youth softball coaches. The camp will cover the very basic softball fundamentals. The cost is $25 each or $20 for two participants. The camp will be held July 14, 2018 10am-1pm. GIRLS’ SUMMER BASKETBALL LEAGUE The City of Forest Park is offering this fast paced basketball league to girls ages 8-17 years old. All games are played in the gymnasium of the Recreation Center. The cost: $45/residents, $55/nonresidents. Team entries will be accepted at a cost of $360 with a maximum of 10 players. Age control date is June of current year. Games start June 2018. Registration for the next season will start in February 2019.

VOLLEYBALL CAMP This s is an introduction to volleyball for kids ages 10-16 years old. All participants will be taught the fundamentals of volleyball while having fun. Preregistration is a must – space is limited to 32 participants. The camp will be held July 21, 2018 9:00am-11:30am. Cost: $15.

YOUTH & TEEN SOCCER CLINIC (Youth) We will be hosting a soccer clinic for boys and girls ages 6-11 years old on July 16-18, 2016. The clinic will run Monday-Wednesday 6:30pm-8pm at the soccer fields. Cost: $10/residents and $20/nonresidents. Registration is July 2-19, 2018- Monday-Friday 9am7pm. For more information contact Willie Freeman at 404-363-2908.

GIRLS’ VOLLEYBALL FALL LEAGUE This program is designed to promote volleyball in our community as well as to give girls game experience, teach volleyball skills, promote fun and to have good sportsmanship. GENTLEMEN OF ETIQUETTE (G.O.E) Fridays – 5pm-6pm Free mentoring program for young adults ages 12 and up. The purpose of this program is to give our youth the information and knowledge they need to conduct themselves as young men. As well as provide them mentorship, hands on activities, and real life application to young minorities while they are still developing. FUTURE PREP FOR TEENS As we know, the youth are our future. Therefore, goal setting is critical. Having goals and knowing what you need to achieve helps you stay on track and focused. This class is to prepare teens ages 13-17 years old for the future. SUMMER 2018 SWIM LESSONS

The Forest Park Recreation & Leisure Services is looking forward to the summer swim lessons and welcomes you and your son/daughter to our Swim Program. We will take registration until the classes are full, Monday through Friday. Registration will begin Wednesday, April 2, 2018 from 9am–7pm at the Recreation Center Gym Office. Payment must be cash, money order, credit card (Visa card, Discover card, or MasterCard). Payment is due at registration. The fee is $40 for residents of Forest Park and $50 for non-residents. A parent or legal guardian must sign the registration form prior to the child participating in the swim lessons. Please make sure that you select the sessions that your child will be able to attend. We will not be able to refund or change classes. The classes will be held in the indoor pool. Fridays will be used, if necessary, for make-up dates due to the Recreation Department canceling of any classes.

4th Session – July 9 through July 19 (8 lessons/2weeks) Class times – 12noon-12:30pm This half hour class adjusts children to the water so they learn to prevent swallowing water by blowing bubbles, to kick while wearing flotation devices, to reach and use arms. Based on the child’s readiness, other skills can be taught, such as: submersion, floating on their backs, climbing out at the side of the pool and jumping in from the side. The above skills are taught through play and games. Parents are asked to not enter the pool area while classes are in session so the child and instructor can fully concentrate on skills to be learned. On Thursdays, the parents can come in to observe and talk with the WSI instructor about any personal concerns.

Fall & Spring Saturday Swim Lessons

Children will be introduced to swim lessons on day one by “Rafty” the duck and friends through story developed by the Red Cross. The lessons will take place in the indoor pool on Saturdays. Registration will start on November 1, 2018 until classes are filled. The fee is $40 for residents of Forest Park and $50 for non-residents. A parent or legal guardian must sign the registration form prior to the child participating in the swim lessons. Each class will accept six (6) participants. This is a must for all Forest Park Recreation and Leisure Services volunteer coaches. The National Youth Sports Coaches Association covers the areas of psy-

chology of coaching youth sports, maximizing athletic performance, first-aid, and safety, how to organize a fun and interesting practice and tips on teaching sports techniques and other important areas. As a member, the coach receives association benefits including liability insurance. Classes will be held at the Forest Park Recreation Center. Registration is required. The cost is $20.
